
About Remote Care
In 2015, Medicare enacted the beginnings of Remote Care on a limited basis to see if the results would match its expectations of reducing hospital admissions and readmissions, reducing ER visits, and decreasing overall Medicare costs. The result then, and since, have been exceptional and Medicare has increased the number of programs substantially.
While we use a number of these programs when appropriate, our Remote Care Today program focuses primarily on the main categories: Chronic Care Management, Remote Physiologic (or Patient) Monitoring, and Remote Therapeutic Monitoring.

Chronic Care Management (CCM) is designed to help individuals with multiple chronic conditions better manage their own health and prevent complications.
The Goals For Patients:

Greater personal involvement in a patient’s own health and well-being.

Improved communication and coordination among their healthcare team.
Requirements: Two or More Chronic Conditions Expected to Last at Least One Year. Most people over 65 qualify for CCM.

Remote Patient Monitoring (RPM) uses technology to collect and transmit a patient's health data, such as vital signs and/or glucose levels, often from the patient’s home.

Patients to use FDA approved and internet connected medical devices to collect and transmit their health data to a provider or clinical care team.

This data is analyzed and used to monitor changes in the patient's condition and adjust treatment plans accordingly.
To qualify for payment by Medicare Part B* and most Medicare Advantage* plans, you need:
Initiating Visit:

There needs to be an “initiating visit” (in person or telehealth) before beginning RPM services for new patients if the patient hasn’t been seen by the provider (usually a PCP) within the last year.
Interactive Communication:

A minimum of 20 minutes per month of interactive communication must take place between the medical provider or a clinical care team and the patient.
Device Usage:

The device that is used for collecting and transmitting patient data must be used on at least 16 days over a 30-day period for Medicare or a Medicare Advantage plan to cover it.

Remote Therapeutic Monitoring (RTM) monitors patients' non-physiological data using FDA-approved medical devices. This data helps track patient's musculoskeletal and/or respiratory systems, medication adherence, and therapy response, among other things. RTM centers on monitoring musculoskeletal and respiratory systems, and has recently expanded to include cognitive behavioral therapy, and focuses on improving, maintaining, or slowly regression.
Key aspects of RTM:

Non-physiological data:
RTM focuses on data that isn't directly related to vital signs, looking at, but not limited to, medication adherence, musculoskeletal activity, and respiratory activity, as well as cognitive maintenance and improvement for balance, mobility and memory.

FDA-approved devices:
RTM utilizes medical devices that meet FDA standards to collect this data. A device, such as the one used by Seniors Home Services, can also be software that provides mental and physical exercises and tracks progress.

Focus on therapeutic outcomes:
RTM helps providers track patient responses to therapeutic interventions, allowing for adjustments to treatment plans as needed.

Such as osteoarthritis, rheumatoid arthritis, loss of balance and its prevention, and other joint or muscle-related issues

Such as COPD, asthma, and other lung-related problems
Cognitive Behaviorial Therapy
RTM can also be used to track adherence and progress in cognitive behavioral therapy programs, including cognitive decline and its prevention.
